Translation Notes:

Y hay algo fuera de lugar, / Que no me dejar claramente, sólo alucinar.

estar mal = to be ill; to be wrong, incorrect; to not be doing well (usually related to health)

Here, the singer clearly means to refer to her well-being, but not specifically her health, so I translated mal to "down" to refer to a general sense of being unwell due to emotional reasons.

Both are correct. The repetition of espero is meant to sound like a repetition of the first phrase, but then becomes a verb for the second phrase.
